Digital ID cards can be accessed 24/7 by creating an account with UHCSR online at uhcsr.com/myaccount or by downloading the UHCSR mobile app to your smart phone or device. To create your account, you will need to use your student ID number.

Undergraduate students taking six (6) or more credit hours, and all Graduate students taking three (3) or more credit hours, and CO-OP students are eligible to enroll. Classes must be on campus; online classes are not applied to the credit hour requirement.

International students are required to maintain insurance coverage throughout enrollment at the University of Louisville. Students need to register/ record their choice to enroll or waive the coverage by specified deadlines. 

Health Professional students are automatically charged for the student health insurance on to their student account. Students need to register/ record their choice to enroll or waive the coverage by specified deadlines. There are no exceptions. 

Graduate Teaching Assistants and Research Assistants (GTA / RA) are automatically enrolled in the plan as part of your benefit package. There is no cost to the student, and this plan cannot be waived.

Continuation of Coverage is only available to Insured Students and covered Dependents who have graduated or are no longer eligible for coverage under the University of Louisville Student Health Insurance Plan. Covered students must have been insured for at least three (3) continuous months before coverage terminated under the Prior and/or Current Plan. Students must enroll within 30 days after coverage terminates.
